Legacy Foundation of Southeast Arizona

Children’s Health Fund (CHF)
Children’s Health Fund (CHF), co-founded in 1987 by singer/songwriter Paul Simon and pediatrician/child advocate Irwin Redlener, MD, is committed to providing healthcare to the nation’s most medically underserved children. CCHCI has been a member of this dynamic national network of pediatric programs since 2008. www.childrenshealthfund.org
Arizona Alliance of Community Health Centers (AACHC)
The Arizona Alliance for Community Health Centers (AACHC) is the Primary Care Association (PCA) for the State of Arizona. All states have one designated PCA to advance the expansion of Federally Qualified Health Centers (FQHCs) and advocate for the health care interests of the medically underserved and uninsured. CCHCI has been an active member of the AACHC since our founding in 1996. www.aachc.org
Women Infants Children Program (WIC)
The Arizona Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides nutrition education and breastfeeding support services, supplemental nutritious foods, and referrals to health and social services. WIC serves pregnant, breastfeeding, and postpartum women; infants; and children under the age of five who are determined to be at nutritional risk. The WIC Program is funded by the United States Department of Agriculture. CCHCI has partnered with WIC to offer in-house services at the Pediatric Center of Excellence in Douglas since 2011. www.cochise.az.gov/cochise_health
Reach Out & Read Southern Arizona (ROR)
Reach Out and Read Arizona (ROR) was established in 2002 and seeks to prepares America’s youngest children to succeed in school by partnering with doctors to prescribe books and encourage families to read together. Reach Out and Read Arizona trains medical providers to integrate literacy into well child visits by gifting new books to children between the ages of 6 months through 5 years. CCHCI has been a ROR participant since 2006, and all CCHCI pediatric sites now offer this program! www.roraz.org/southern-arizona.asp
Douglas Unified School District (DUSD)
Douglas Unified School District (DUSD) operates eight schools in the City of Douglas. DUSD believes that “through the combined efforts of involved parents, devoted teachers, dedicated staff and administrators, and enthusiastic students, every student can reach his or her highest potential” CCHCI Pediatrics collaborates with DUSD in multiple formal and informal ways, from collaboration with school nurses for children with special health care needs to participation in educational sessions. In 2014, in an effort to improve our outreach to adolescents, CCHCI established a formal agreement to allow for provision of on-site adolescent medical care via the Mobile Medical Clinic. www.dusd.k12.az.us
Southern Arizona Komen Foundation
The Southern Arizona Affiliate of Susan G. Komen for the Cure was started in 1999 and focuses on making grants for clinics that treat uninsured and underinsured women. Since 2006, CCHCI has been receiving assistance to provide screening, diagnosis and treatment of breast cancer for our most needy patients that are uninsured and do not qualify for any other programs. www.komensaz.org
University of Arizona Zuckerman College of Public Health
As Arizona’s first and only accredited college of public health, the Mel and Enid Zuckerman College of Public Health offers tremendous opportunities to its students for education, research and community involvement thanks to a locale rich in history, research opportunities, and university academic resources. CCHCI has received episodic funding from College of Public Health’s Rural Health Outreach Program for various binational border events. publichealth.arizona.edu
Mexican Consulate
The Ventanilla de Salud is a free program funded by the government of Mexico designed to assist recent immigrants in navigating U.S. health services. The primary goals are to prevent disease, to promote healthy habits and to refer you to health services. Since 2010, the Mexican Consulate has participated with CCHCI in the Ventanilla de Salud program to assist Mexican Citizens in our area to navigate the health services available in our community. ventanillas.org/index.php/en
Adelante Community Health Center
Adelante Community Health Center is a Federally Qualified Health Center that operates health centers throughout central Arizona, offering a wide array of services to patients of all economic backgrounds. Since 2010, CCHCI and Adelante CHC collaborate to provide screening and treatment services to the Migrant and Seasonal Farmworkers (MSFW) in the Yuma area. Our MSFW team screens the farmworkers in the fields and refers those needing medical treatment to Sunset CHC for the provision of medical services. www.adelantehealthcare.com
Douglas Association for Retarded Citizens (DARC)
Douglas Association for Retarded Citizens (DARC) has provided habilitation, employment, transportation, social integration and residential services for individuals with disabilities living in the Douglas, Arizona area. Since 2011, DARC clients assemble packets that are utilized in our Migrant and Seasonal Farmworkers Program, and are financially compensated for their work.
Southeast Arizona Health Education Center (SEAHEC)
Since 1985, SEAHEC has been working to expand the reach of health-related services to those places where medical resources are most limited or lacking. SEAHEC helps to strengthen the wellness of rural and medically underserved communities by developing health care providers and health care programs. CCHCI and SEAHEC have collaborated on a project called “Healthy Farms” since 2011. The project recognizes farms that promote safety for the farm workers and also encourages advocacy efforts and worker safety practices amongst the agricultural community. www.seahec.org
Chicanos Por La Causa (CPLC)
CPLC is the only community development corporation in Arizona that offers extensive services in both urban and rural areas. Its service delivery system is expansive enough to effectively reach thousands of low-income people. In its perpetual drive to provide needed services, CPLC continually assesses the needs of its communities throughout Arizona. Beginning in 2014, the CCHCI dental team is works with Chicanos Por La Causa to provide uninsured children in the Willcox area with dental exams and needed care. www.cplc.org
Sierra Vista Regional Health Center (SVRHC)
Formed in 1964, SVRHC is Cochise County’s largest and most comprehensive tertiary health center. Beginning in 2014, SVRHC has provided space for a CCHCI Outreach & Enrollment Specialist to carry out insurance eligibility, enrollment and referral for uninsured/underinsured patients. sierravistaregionalhealth.com
